Limited Benefit Health Options
The Montana Nonprofit Association (MNA) is pleased to present nonprofit members of MNA with expanded options for employee benefits. MNA members are able to work with Peak1 Administration (formerly Insurance Coordinators of Montana) to offer Select Benefits limited benefit medical insurance from Symetra Life Insurance Company to their employees. In addition, our members can work with Peak1 to purchase Vision Coverage, Life Insurance, and Dental Insurance as well. Since these coverages are now being offered ala carte you can offer any of them alone or in combination to you employees. For MNA members choosing two or more of these coverages Peak1 will waive the $250 fee to set up a Premium Only Flex Plan that will save your organization and your employees even more money.

Program Options
Select Benefits Limited Benefit Health Plan
Select Benefits coverage starts at about $60 per month and offers cost-effective limited benefit medical coverage to your eligible employees; their spouse/partner and dependents are covered at no additional cost. Employment may be full-time or part-time. Select Benefits has flexibility not found in most other limited benefit plans, including no networks, no preexisting condition limitations, no deductibles, and no conflicts with existing insurance. Select Benefits is not a replacement for Major Medical insurance.
Ameritas Group Dental Insurance Plan
Ameritas Dental coverage starts at $32 per employee per month and has a $50 deductible that is waived for all “Type I” benefits including up to two routine exams or cleanings per year. Our plan pays up to 90% of locally Usual and Customary price for procedures and does not require your dentist be a part of a network. In addition, our dental plan allows annual carryover of unused maximum (to a maximum of an additional $1,000) to help you plan for more expensive procedures.
UNUM Life Plan
UNUM Life coverage is available in the amount of up to 5 times salary to a maximum of $ 500,000 (with a minimum of $10,000), for as little as $1.21 a month for a 35 year old wanting $10,000 of Life coverage. Accidental Death & Dismemberment coverage can be purchased for yourself regardless of whether you purchase Term Life coverage.
VSP Vision Insurance
There are 3 options available for each member to choose from. For as little as $2.60 per employee per month, the employee and their entire family can receive annual eye exams with no co-pays. VSP can also provide up to 20% discount on all vision services to those that enroll.
Eligibility
To participate in the this program, your organization must be a 501(c)(3) organization and a member of the MNA. Other restrictions may apply, please ask yourPeak1 representative to explain each program in full detail.
